## Name the action

- Request ID and version:
- Acting identity and represented user/team:
- Exact operation and complete reviewed parameters:
- Destination/target, environment, and affected records:
- Data leaving the system (including queries, attachments, and logs):
- Expected direct effects and known downstream effects:
- Existing permission and policy version:

## Classify it

- ALLOW: within narrow, current permission with understood effects.
- ASK: permitted in principle but requires a specific authorized decision.
- DENY: prohibited or outside the actor's authority.
- Incomplete information: pause, gather details, and classify again.

Chosen decision and reason:

## If asking

- Authorized reviewer identity/role and scope:
- Complete action the reviewer will see:
- What one approval covers (one action, bounded batch, or other explicit scope):
- Expiry and rationale:
- Changes that invalidate this decision:
- Policy/authority revalidation immediately before execution:
- Denial, timeout, and revocation handling:

## Observe execution separately

- Decision ID, reviewer, timestamp, scope, and action version:
- Execution attempt ID and downstream deduplication mechanism:
- Concurrency control for competing workers:
- Observed result (acknowledged / failed / unknown):
- Reconciliation procedure for an unknown result:
- Redaction, access, retention, and tamper protection for evidence:

## Proposed acceptance checks

Test unchanged approval, denial, expiry, changed recipient/content, unauthorized reviewer,
concurrent workers, ambiguous provider timeout, revocation, and changed policy.
Record actual commands, integration versions, results, and limits separately.
Do not label a planned check as a passing test.
